This report exposes the frequent use of force based on racial bias and prejudice that tends to result in unjustified killings of African Americans. This systematic failure is represented on the cover art by a tombstone which is also a shooting target, the same that police use for shooting training. The target is surrounded by hands trying to stop the bullets, hands asking for help, and hands expressing suffering and pain over the unjustified loss of human lives.
